Admission Criteria
We are a state-licensed day habililtation program. Much of our admission criteria is set by the TN Department of Disability and Aging.
To keep in compliance with our state license, all our participants (Pals) must meet the following minimum requirements:

be 18 years of age or older

have a primary diagnosis of Intellectual Disability, Developmental Disability, or Autism

meet their own personal needs (feeding and toileting)

be able to function well in a 7 Pal to 1 Staff ratio.

the needs and behaviors must be manageable enough to not require one-on-one assistance.

have had a medical exam within the last 6 months which shows he/she is free of communicable disease.
Jeremiah's Promise admits individuals of any race, color, religion, national and ethnic origin to all programs and activities.
Tracks and Tuition
There is a $50 non-refundable application fee due when the application is turned in.
We have the following tracks available (depending on openings):
2 days/week, Track A: Monday and Wednesday
2 days/week, Track B: Tuesday and Thursday
3 days/week, Track C: Monday, Wednesday, and Friday
3 days/week, Track D: Tuesday, Thursday, and Friday
4 days/week, Track E: Any four days of the parents' choosing
5 days/week, Track F: Monday through Friday
Our regular hours are 9:00am - 3:30pm.
We have extended hours from 7:45-9:00am and 3:30-4:30pm for $6 for each hour needed.

Tuition Rates:
2 days/week: $280/month
3 days/week: $420/month
4 days/week: $560/month
5 days/week: $700/month
Fridays are Life Skills focus days. The Pals work on daily living skills including menu planning and grocery list making. They also cook/prepare lunch and eat what they prepare. The cost is $7 for each Friday lunch. Pals are welcome to bring their lunch on days they don't want to eat what is on the menu.

After going over our admission criteria, caregivers and parents will have an idea if we can meet the individual needs of the applicant. If there are any questions, reach out and we will explore this together!

If, after reviewing our admission criteria, you feel you would like to pursue admission for your adult child/loved one, reach out to set up a tour.
We will take you on a tour of our facility, let you meet our staff, and allow you to see our program in action. After that we will do a brief interview and allow you to ask any questions you have.

Fill out the online Application.
If, after your visit, both parties agree that this might be a good fit, an application packet will be given at the end of the visit. There is a $50 non-refundable application fee due when the application is turned back in.

Admissions Team Recommendation
After the application is submitted, the admissions team will review it. They will make a recommendation. We ask for two weeks to allow the team to review the application packet fully. We will then notify the family.
